Commissioner of Oaths

The Township of Scugog’s Corporate Services department provides Commissioner of Oaths services for documents such as affidavits, oaths and statutory declarations. All parties required to sign the document must attend our office, in person, with a valid piece of government issued photo identification. Copies are not accepted.  Documents should not be signed until you are in the presence of the Commissioner of Oaths.

We cannot guarantee that we can commission documents until reviewed by the Commissioner of Oaths.  We reserve the right to refuse commissioning services of any document.

A Commissioner is available during regular business hours at the Municipal Office, 181 Perry Street, Port Perry, Monday through Friday from 8:30 a.m. to 4:30 p.m. (excluding holidays).

Fee:
$27.00 (with tax $30.51) for up to 5 signatures
($7.00, plus HST, for each additional document)
Payable by cash, debit or cheque (No credit card payment)

  • Proof of Life – Domestic and Foreign Pension Documents (no fee)
  • Sworn Statement of a Family Gift of a Used Motor Vehicle in the Province of Ontario
  • Sworn Statement for the Transfer of a Used Motor Vehicle in the Province of Ontario
  • Statutory Declaration of Name Change (booklet must be completed prior to commission)
  • Statutory Declaration for OSAP – Statement of Common Law Status/Supporting Children

  • Wills, “Living” Wills, Codicils to Wills
  • Powers of Attorney
  • Divorce, Separation or Cohabitation documents
  • Custody documents
  • Statutory Declaration of Progress Payment Distribution – Holdback Security Deposits (Subcontractors)
  • Real Estate documents, including transfer of land
  • Estate Settlement Documents
  • Court, Legal and/or Civil Issue related documents
  • Documents related to Finances (Banking information, RRSP, etc.)
  • Documents requiring signature of a Notary Public
